Rail defect missed: Safety analysis not flagged before derailment, probe finds

A RAIL defect that “significantly exceeded” criteria was identified before two Pacific National locomotives pulling 20 wagons derailed near Ouyen, an investigation has found.

The Australian Transport Safety Bureau (ATSB) yesterday released its findings into the November 2015 incident, which found a safety analysis for the stretch of line was not flagged with track supervisors in the months before the accident.

The Pacific National train was travelling from Carwarp to Melbourne with 19 of the 20 wagons loaded with grain.
